By popular demand, this is a PDF copy of the meeting notes/action list form we have available for clients and families to take notes and list action steps during their appointments. It is a handy form for anyone who wants to keep up with actions assigned during any type of meeting.

Use This Tool During Meetings
-
When attending a meeting, write the date, purpose of the meeting, and initials of the attendees at the top of the page.
-
Take meeting notes on the large left column.
-
When you or someone else agrees to or is assigned to take a specific action, document this in the three columns on the right side of the page. (1) number the task in the first right column, (2) write the initials of who will complete the task in the second column, and (3) document the date the task is to be completed (when) in the third column.
-
Continue numbering action items (in order) while recording appropriate information in the who and when columns.
-
At the end of the meeting, record the date for your next meeting or follow-up appointment.
-
After the meeting, review your notes and transfer the tasks you’ve agreed to complete to your planner or your list of tasks to be completed at work.
-
Record the date for your next meeting in your planner of choice.
